Linking data in Cheshire and Merseyside

We are working to bring together (or 'link') data from services across Cheshire and Merseyside. This will help health and care organisations in Cheshire and Merseyside understand the needs of the local population and inform and improve services in the region.

This introductory guide explains how the ACG System identifies patients with certain risks and how that affects your population health strategy. It covers the fundamentals of risk stratification and how it supports proactive, data-led care.

This e-guide provides a framework to use electronic health record (EHR) data to provide risk analysis and glean insight for improved care. It explores commonly occurring challenges within a health system or provider organisation and presents corresponding use cases to solve those challenges.

This document sets out the purpose, membership, and responsibilities of local place-based Population Health Management (Proactive Care) Groups in Cheshire and Merseyside. These groups will take a holistic view of the system and provide strategic direction to the development of data led models to have optimum impact on population outcomes. The terms of reference explain how the groups work, who is involved, and how they contribute to joined-up, proactive care in the community.

This guide explains how to successfully implement data-led and risk-stratified models of care across local services using linked data across Cheshire and Merseyside. It covers the end-to-end rollout process, from onboarding and training to governance and ongoing support. It is intended for programme leads, operational teams, clinicians, analysts, and all stakeholders involved in the adoption and use of linked data tools.

Current research in progress in Cheshire and Merseyside

The data-sharing register lists all research projects that are approved to use health data through the Cheshire and Merseyside Secure Data Environment.

It shows what researchers are doing with the data so patients and members of the public can feel confident it is only being used in the right way and for the right reasons.
